Person-centered planning is a key element to Integrity Nursing. Person-centered planning is a way to help people supported plan services based on their needs and desires. The individual should be the focus and the center of decision-making during healthcare planning. Integrity Nursing will work diligently with a multidisciplinary team and use all tools available, including the LSP and electronic record, to provide the best possible healthcare. The individual and legal guardian (if applicable) will be fully included and informed of all aspects of the healthcare of the individual. The purpose is to ensure the best possible health of the individuals so they can more fully develop collaborative supports focused on community presence, community participation, positive relationships, and respect. 

Integrity Nursing will focus on being able to answer these four questions about the individual:

1. What is important to the individual?

2. What is important for the individual?

3. Is there a good balance between "important TO" and "important FOR?"

4. What does the individual want to learn, what do we need to learn?
